Withdrawal Policy:

• I understand that there are no refunds for paid tuition.
• I understand that I must withdraw my child from Creative Connexions Performing Arts Center via THIS FORM ONLY and that: 
  1. Not attending classes does not imply notice of withdrawal.
  2. Communicating withdrawal in person will not be accepted.
  3. Communicating withdrawal over the phone, voicemail or text message will not be accepted. 
  4. Communicating withdrawal via social media channels will not be accepted. 
• I understand that if I want to stop the automatic tuition on the 20th of the month, I will withdraw at least 72 hours in advance via this form (by the 17th of the month).
